Hi Katherine- I want to ask you a few follow-up questions regarding your health history form.
How hard do you have to exert yourself before you have shortness of breath? Is it after extensive physical activity or do you experience it after walking upstairs or to your car?
I am so sorry to hear about the death of your husband. How are you coping? I noticed that you were hospitalized for angina around the time of your husband's death. Are these events related? Have you had any other episodes? What kind of stress, if any, aggravates your condition? Do you always carry your nitroglycerin with you?
You stated that your TMJ disorder gives you headaches, head and neck problems and that you have difficulty opening your mouth wide. Do you take medication when your TMJ troubles you? Do you have trouble staying open at your dental appointments?
I noticed that you had a visit with your physician 2 months ago. Was that for your regular check-up or were you having any problems?
How is your partial feeling? How often do you wear your partial? How do you take care of your partial?
Tell me how you take care of your bridge.
It says on your HH that you had a recent toothache? What kind of symptoms were you having? Did you see a dentist regarding this?
How often do you have hot/cold/pressure sensitivity? Is it pretty generalized or do you notice sensitivity of any teeth in particular? Is it just hot and cold sensitivity or pressure too?
I noticed that you have a couple deep pockets. Have you ever been told that you have gum disease? Have you ever had a deep cleaning or a root planing?
Do you ever trap food between your teeth?
You say that you have trouble chewing. Are there particular foods that you have trouble eating? What are you experiencing- pain, sensitivity? Are you having difficulty because of your partial?

Climbing steps will cause me to have shortness of breath.
I believe the angina was probably related to my husband's death. I did use the nitroglycerin about 1 year ago. I have not needed it since so I have not been carrying it with me. I really don't remember what brought the angina on it was so long ago.
I will take the Tylenol when I get a headache. It seems to be related to when my jaws get sore and I clench my teeth. I do have trouble keeping my mouth open for long appointments.
Last doctor appointment was a routine check on my status.
My partial seems a bit loose but not too bad. I wear it all the time--I sometimes take it out at night--if I remember. When I take it out I soak it in Polident.
I clean under my bridge with toothpicks because food gets caught under it. I have a lot of places where food gets caught.
My teeth are sensitive to cold and hot too--I think--mostly cold though--- and brushing. That is why I am here besides needing my teeth. cleaned. I don't notice pressure sensitivity too often--sometimes my teeth ache in the morning when I get up.
I had my teeth deep cleaned at my last dentist--Boy did that hurt. I didn't want to go back to that place. Does it have to hurt so much when you get your teeth cleaned?
When my teeth are sensitive in the morning it hurts with cold milk on the cereal and sometimes chewing the ceral is painful. It is kind of gereralized discomfort. It is not like one tooth is throbbing. They all seem a bit tender.
I don't think it is related to the partial.
