expense of dental treatment
You mentioned earlier that you had several of your teeth extracted because it was more financially manageable that paying for root canals. Is that what especially brings you to U of M today, that treatment here is more cost effective?
We are happy to serve you and we hope we can accomodate your needs for what you can afford.

A few questions...
I took a look at your x-rays. Is that your normal bite? Is your bite stable?
Where is your arthritis? Does it bother you often?

Hi Katherine-
Your blood pressure today is 103/72. Is this a normal blood pressure for you?
How did your last check-up go? Did your physician make any comments on your health status?
Also- you said you never premedicated for any dental treatments. Has this ever been discussed with your physician as being unnecessary?

Hi Katherine-
I also wanted to ask you about your missing teeth. Why did you have those extractions? Have you had any other oral surgeries? If so, what were they?

Premed
Hi Katherine-
I have another question for you.
Has a physician ever told you that you need to premedicate prior to dental procedures because of your rhematoid arthritis?

Hi Katherine- I want to ask you a few follow-up questions regarding your health history form.
How hard do you have to exert yourself before you have shortness of breath? Is it after extensive physical activity or do you experience it after walking upstairs or to your car?
I am so sorry to hear about the death of your husband. How are you coping? I noticed that you were hospitalized for angina around the time of your husband's death. Are these events related? Have you had any other episodes? What kind of stress, if any, aggravates your condition? Do you always carry your nitroglycerin with you?
You stated that your TMJ disorder gives you headaches, head and neck problems and that you have difficulty opening your mouth wide. Do you take medication when your TMJ troubles you? Do you have trouble staying open at your dental appointments?
I noticed that you had a visit with your physician 2 months ago. Was that for your regular check-up or were you having any problems?
How is your partial feeling? How often do you wear your partial? How do you take care of your partial?
Tell me how you take care of your bridge.
It says on your HH that you had a recent toothache? What kind of symptoms were you having? Did you see a dentist regarding this?
How often do you have hot/cold/pressure sensitivity? Is it pretty generalized or do you notice sensitivity of any teeth in particular? Is it just hot and cold sensitivity or pressure too?
I noticed that you have a couple deep pockets. Have you ever been told that you have gum disease? Have you ever had a deep cleaning or a root planing?
Do you ever trap food between your teeth?
You say that you have trouble chewing. Are there particular foods that you have trouble eating? What are you experiencing- pain, sensitivity? Are you having difficulty because of your partial?

Caries history, Fluoride, Diet, Self-care
Do you have any history of cavities?
Do you have fluoridated water at home?
What types of foods do you prefer for snacking? What do you drink when you're thirsty?
What type of toothpaste do you use? Do you know if it contains fluoride? Is your toothbrush one with hard or soft bristles?

Tylenol, Dry Mouth, and Premarin
Hi Kathryn,
I just have a few questions for you regarding your medical and dental histories.
Do you take Tylenol for arthritic pain? And are there other reasons you take tylenol?
You said that you don't experience dry mouth. That's interesting because several of your medications indicate dry mouth as an oral side effect. Have you ever experienced an issue with dry mouth that you know of?
Do you know why you are taking Premarin?
