Kwan,

How are you doing today? I want to ask you just a few more questions regarding your medical history. Have you ever had bronchitis or any other breathing problems besides your emphysema? Have you had pancreatic disease or any other hormonal problems? Lastly, have you ever taken any drugs for bone loss?

Brushing/Flossing

Mr Kwan-
You have mentioned that you brush very irregulary and don't know how to floss. Do you have difficulty holding a toothbrush/floss, does it cause you pain to do either?

There are a lot of products such as power assisted toothbrushes and many floss innovations designed to ease the process, I hope we can find something that will help you out and make it easier to care for all those great natural teeth of yours.

typical day

Mr Kang-
It sounds like you have a lot of good stories to share with your military history and job as a mechanical engineer!
Could you please tell me what your typical day is like now that you have retired? How much time do you spend outside?
I was also wondering how long has it been since you have stopped smoking?

paperwork questions

Mrs. Gwozdek,
Should supplemental oxygen be noted on the white drug reference form? I was having trouble finding anything in the drug book about oxygen.

Also, what date should be used for the currrent date on the treatment form?

Extra Questions

Hello Kwan,

How are you doing today? I want to clear a few things up with you regarding the information from the Medical and Dental history form. Have you ever had Bronchitis or any other breathing problems besides emphysema? Have you ever had adrenal or any other hormonal gland disease?

I see that you didn't fill out most of the section that has to do with heart or circulation problems. I would like to ask you the questions that you left black again. Have you had any problems with irregular heart beats, heart attacks, rheumatic fever, mitral valve prolapse, any type of damages or problems with your heart valves, swollen limbs or congestive heart failure? Lastly, were you ever dependent on a pace maker or any other cardiac divice?

I would also like to ask you a few other questions regarding this same history form. Have you had any other kidney problems other than kidney disease and have you been on dialysis? Have you ever had hives, skin cancer, back problems, or any other muscle, bone,or skind disease? Have you had any problems with your stomach or liver?

Also, do you have mutiple sclerosis? Are you taking any blodd thinner medications, had chemotherapy/radiation treatment, or any other problems with your blood or immune system?

Endodontics

Mr. Kang,
looking through your x-rays I was wondering if you ever had any root canals done on your teeth? Or if you have had any tooth abscess? Abscess is just a collection of pus that can be caused either by decay or tooth trama.

Allergic Reactions

Hello Mr. Kang,
How are you feeling today? I have an important question to ask you. I was wondering if you are allergic to anything and/or have had allergic or unusual reactions to local anesthetic? Such reactions could be hives, itching, bumps, redness, swelling, etc?

medical history

Hi Kwan Kang,
Hello Kwan,I was looking over your medical history and I had a few more questions to ask before it is complete. I was wondering how your emphysema currently is? That is wonderful you stopped using tobacco! I notice you are retired, do you feel like you are less stressed now because grinding your teeth can be stress related, and do you have a bite splint? How long have you had your tooth ache for? are you sensitive to hot or cold? Does it hurt when you bite down or chew? Can you pin point what tooth is giving you the problem? I would love to help you resolve your problem.

Industrial Engineer

Mr. Kwan,

Upon reviewing your health history form I noticed that you are a retired industrial engineer. Can you tell me more about your job: how long you worked in this profession, your daily schedule, how long ago you retired, job duties, etc? Did you ever experience any job related stress? I ask this because stress can alter your oral status, as well as the rest of your body, depending on the severity and length of the stress. This information will help me assess your risk for periodontal disease.

I have another question for you. When I conducted the intraoral exam, I noticed there was a small white sore on the under side of your tongue? Can you tell me anything about that? Is it causing you any pain or discomfort? Can you tell me how long it's been there?

Family History

I have a few more questions that relate you habits and oral history. Do you have a family history of perio disease or oral skin cancer? Also, how well do you protect yourself from the sun? Now, I know you mentioned your brushing habits, but can you explain or show me how you brush your teeth whether it's in a circular motion?

Dental History Information (Pink form)

Hello Mr. Kang!

After looking over your medical history that you completed today, I have a few questions for you. Do you need any special accommodations for your dental treatment such as sitting in a certain position for your emphysema? Also, do you have any issues in your life that cause everyday stresses or elevate your stress in anyway? I noticed that you grind your teeth. Have you ever worn a bitesplint/nightguard? Also, Is there anything we should know about your jaw, such as, popping or clicking when chewing, discomfort or difficulty opening your mouth, or injury to mouth or face in the past? If so, have you ever experienced surgery in your mouth or on your lips?

Emphysema

Dry Mouth/ Pain associated with toothache

Hello Mr. Kang,

How are you doing today? I wanted to discuss your situation with dry mouth. How long have you had this problem? Does anything make it better or worse?

I also wanted to hear more about the toothache you seem to be experiencing. How long have you been tolerating this pain? Could you describe the intensity of the pain on a scale from 1 - 10, 10 being the most intense? I would like to help you deal with this concern.

raised lesion on left temporal region

Diet

Kwan,

What does your diet consist of? If you consume sugar, how often do you eat it? Do you eat it all at once, or do you snack on it throughout the day? Lastly, what type of foods do you eat that consist of sugar or high carbohydrates?

Missing Teeth

Kwan,
Through looking over your dental history and exam forms I have seen that you are missing all of your third molars, also known as your wisdom teeth. I was curious to know if you had them removed, or if you were just born without them? If you did get them removed do you know when that took place? Hope your day is going well!
