Survivorship Care Plans
Hi Volunteers,
The new toolkit refers patients to the PERC to find samples of a Survivorship Care Plan. You can find sample Survivorship Plans online at the ASCO site:
http://www.cancer.net/patient/Survivorship/ASCO+Cancer+Treatment+Summaries
This ASCO site has a generic plan and also a few plans that are specific to different cancers.
In addition, on the Lance Armstrong LIVESTRONG site, a person can create their own personal plan through an online form. The URL is:
