Field Evaluation Due Date - April 20

Field Evaluations are due to the Field Office on Monday, April 20. Please be sure to include the original 'Term Hours' page, student self-assessment, and field instructor narrative, all with original signatures.
Remember to make a copy of the complete application, as you will NOT be able to borrow it for copies once it is turned in for grading. Please contact your Field Liaison or the Field Office if you have any questions.
