Agency seeks professionals with genuine interest in services to elderly. Working in a team environment you will conduct assessments, develop client care plans, monitor and report on care provided.
Requirements:
If you are minimum licensed RN / BSW Social Worker, and have experience in a Home Health Care setting or as a Care Manager working with a geriatric population, we are looking for you. Some fieldwork is required. Full Benefits, 37.5 hours/week, weekdays only; EOE.
