Hope Network provides a continuum of specialized care for those with brain and spinal cord injuries, mental illness, developmental disabilities, substance abuse disorders and other disadvantages. Our support services include transportation, subsidized housing, case management, community-based services and workforce development.

The Case Manager is responsible for the development, implementation and monitoring of the person-centered plan to assure that outcomes, tasks and activities reflect consumer strengths, needs and desires; to assure that consumer rights to treatment are delivered in the least restrictive setting; and that consumers have access to community based services. Competency in carrying out psychosocial assessments. Maintain consumer records-meeting all appropriate standards from HIPAA trainings including confidentiality at all times
- Current Michigan driver's license with acceptable driving record.
- High School Diploma required
- Must have a BSW
- Prior case management experience in a mental health or rehabilitation setting.
Case Manager I
First Shift 40 Hours per Week
Entry-level starting at $30,500/year. Greater depending on experience.
