Hope Network has an immediate opening for a Case Manager at its office in Jackson. This position is responsible for the development, implementation and monitoring of the person-centered plan to assure that outcomes, tasks and activities reflect consumer strengths, needs and desires; to assure that consumer rights to treatment are delivered in the least restrictive setting; and that consumers have access to community based services. Competency in carrying out psychosocial assessments. Maintain consumer records-meeting all appropriate standards from HIPAA trainings including confidentiality at all times.
Additional Qualifications: Must have a BSW and prior case management experience in a mental health or rehabilitation setting.
