Housing Support Services Team Member/CSTS Supports Coordinator
Full-time position available. Housing Support Services works to support Avalon tenants in the successful achievement of housing permanency and satisfaction in their personal lives through the use of timely, on-going, and proactive professional and peer support.
The Team Member/CSTS coordinator will work directly with Community Mental Health consumers living in Avalon Housing. Position is responsible for completing clinical assessments, developing and maintaining Individual Plan of Service (IPOS), coordinating and monitoring client services, directly assisting consumers in achieving desired outcomes, and documenting client progress.

Requirements include a Bachelor’s or Masters degree in social work (must possess limited license, full license preferred), at least one year of experience providing intensive case management to individuals with a mental illness, and prior experience with low-income families and homeless populations a plus. Other requirements include scheduling flexibility, creative problem solving skills, excellent decision making abilities, compassion, and self-motivation.
