Tooth Whitening and How to Find Out About It

We received a comment on this blog today.

"Found this website, when I was looking for tooth whitening dentists (albeit a cheap one). But my question is really do the tooth whitening thing works. Can get that hollywood smile."

This is a very common question these days, one that concerns a lot of folk. Now, since we're librarians and not dentists, I am going to talk mostly about how one could search for this topic of interest, just highlighting a few interesting tools and techniques.

Most folks would start with Google. The question is what would you search for? Whiter teeth? Hollywood smile? Teeth whiteners? Tooth whitening? One of the hardest parts of any question is the time spent finding the right lingo to get at the answer you really want. In this case, it turns out that the phrase used by most of the professional sites in "tooth whitening" or "teeth whitening". Using either of those two in Google will take you to this page by the American Dental Association.

Oral Health Topics: Tooth Whitening Treatments:
http://www.ada.org/public/topics/whitening_faq.asp

This is good stuff. A nice tight overview that touches on the most important issues. What if it isn't quite what you wanted to know? If you wanted to know a little more, or a different level of information?

One nifty search engine for getting deeper into the medical internet is MedStory.

MedStory: Tooth Whitening: http://www.medstory.com/app?service=external&page=Search
&tc=r1&q=tooth+whitening&s=Web&af=true&c
=false&i=

In MedStory you can zip right in to good info by doing your search, clicking on the research tab, and then click on research articles. Good stuff, right off the bat, really easily.

Now, if you REALLY want the BEST information, you are going to look for systematic reviews on a topic. There are a few good ways to do this. One is to go to Cochrane, the gods of systematic reviews. Another way is to go to PubMed, and search your topic with systematic review.

If you go to Cochrane.org, navigate to the Oral Health group and look for toothe whitening, you will find this.

Cochrane: Oral Health Group: http://www.cochrane.org/reviews/en/topics/84_reviews.html

Hasson H, Ismail AI, Neiva G. Home-based chemically-induced whitening of teeth in adults. http://www.cochrane.org/reviews/en/ab006202.html

If you go to Pubmed, to find systematic reviews, go to the bluebar on the left side, click on Clinical Queries, scroll down the screen to Systematic Reviews, and then add you search your topic. If you do that, you'll find this.

Hasson H, Ismail AI, Neiva G. Home-based chemically-induced whitening of teeth in adults. http://www.ncbi.nlm.nih.gov/sites/entrez?Db=pubmed&Cmd=D
etailsSearch&Term=17054282%5Buid%5D

Assuming this is the best article on the topic (which it is, in my personal humble opinion), you can read the abstract and find that the evidence is not yet clear on this topic. It seems to work most of the time, but often causes pain to the teeth and gums. But don't trust me, go read the article. :)

Chemical Blog Aggregator ... and More!

A comment on an earlier post brought to my attention this really interesting aggregator for the Chemical Blogspace, appropriately named "Chemical Blogspace".

Chemical Blogspace: http://cb.openmolecules.net/

"Chemical blogspace collects data from tens of scientific chemistry blogs and then does useful and interesting things with it.
With Chemical blogspace, you can:
* Find, read and subscribe to new science blogs
* Find out what scientists are saying about the latest books and papers
* Read mini-reviews, conference reports or even original research
* See the buzz surrounding different websites
* Browse different subject areas - chemoinformatics, organic chemistry, ... see the 'Explore' options on the top right hand side of the page for more. "

It is the "interesting things" part of this that intrigues me. It includes molecular models, and allows you to browse the blog posts by chemical structure. Not being a chemist, I clicked on a shape that looked interesting to me. It turned out to be hydrocodone, more popularly known as Vicodan.

Chemical blogspace - Molecules: hydrocodone: http://cb.openmolecules.net/inchi.php?id=282

This pulled up two interesting posts that would normally not have been juxtaposed - one about an celebrity overdose death and the other about Chinese traditional herbal remedies. Wow! Now, tell me how else you find that there was a relationships between those dissimilar postings? Very interesting, and I could see this being a real timewaster for someone really into chemistry, biochemistry, and/or pharmacy.

Dentistry Library Transitions Mean Blog Transitions!

Those who watch this blog closely may have noticed that we changed the title of the blog:

FROM:
Dentistry Library

TO:
Dental Information & Library Innovation

This name reflects some of the exciting transitions we will be undergoing over the next few months. I can't say it better than it was said by the Dean, so I have requested permission to share with our blog readers his message to the School community.

-----------------

To the School of Dentistry Community:

This communication reflects the announcement and discussions at the faculty
meeting today that included Paul Courant, University Librarian & Dean of
Libraries, and Jane Blumenthal, Head of the Health Sciences Libraries.

There will soon be changes occurring in many of the libraries on the
University of Michigan campus. This will affect the Dentistry Library and
how it has been operated.

As demand and interests shift from print to electronic resources, the need
for a physical presence and onsite archiving of print materials lessens.
The University Library System and the School of Dentistry have agreed that
the physical space occupied by the Dentistry Library will no longer be
needed for archiving the print collection. Ongoing costs and utilization
assessments can no longer justify maintaining the physical space in the
School of Dentistry. This issue is not unique to dentistry or to the U of M
and is manifesting itself in nearly all disciplines and in most if not all
institutions worldwide.

With this change in the library structure, the dental librarian's role and
title will change to that of informationist. The informationist will have
an office in the dentistry building and will work with faculty, staff and
students on teaching, research, grants, informatics, community outreach and
special projects.

The collection will be removed from the Dental Building. The schedule for
this has not yet been determined, but we will keep you informed. We will
maintain access to books and journals and will continue to provide a full
range of library services, including electronic document and print materials
delivery services.

A combined School of Dentistry/Health Sciences Library task force will
facilitate the transition to a digital information resource by identifying
facilities and services that must be implemented as an interface to the
digital information resource. The task force will hold open meetings to
discuss these needs and any other issues to ensure that the transition
occurs smoothly.

We appreciate the essential role Patricia Anderson has played in the Dental
School and her contributions to its teaching and research. Patricia will
now focus on emerging technologies. She has been reassigned to the Taubman
Library, and I know that she will flourish in that role.

We hope to have the Twitter entries display on our home page when it is revised (process to begin later this month). In the meantime, we will occasionally list our recent Twitter entries in a blog post, for you to explore as you wish. So, you can either check out our "Twitterstream" or just keep watching our blog. Sooner or later, the information will come to you.

What to do with all that Halloween candy?

Dr. Terry Preece in Utah has figured out what to do about all his patients and the Halloween candy they pick up. He buys it from them at a dollar a pound!

He isn't the only dentist doing this. Across the country, countless dentists are engaging in similar programs to encourage children to reduce the consumption of Halloween candies. Closer to home, Dr. Paul W. Allen from Saginaw and Dr. Kevin Flood from Grand Rapids are doing the same thing.

Dentist's sweet deal tops half-ton of candy: http://www.mlive.com/news/saginawnews/index.ssf?/base/news-24/1194020567306600.xml&coll=9

Other folks are also coming up with ideas for how to use up all the worrisome bounty. The ideas include:

buy it from the kids,

hide it,

throw it out,

share it with neighbors,

take it to work,

cook with it,

use it in art projects,

save it for goody bags at a party,

stuff a pinata for your child's next party,

use it to barter for good behavior, or

as a homework reward.

Five ways to use all that candy: http://specialchildren.about.com/b/2007/10/31/five-ways-to-use-all-that-halloween-candy.htm

A rather creative approach is sharing candy with the Pumpkin Fairy (mysterious donations outside your door) or the Haggle Fairy (parents bartering change for candy) as mentioned in this article from San Francisco.

One idea that has really gotten attention this year is building a gingerbread house from Halloween candy. The famous FLYLady recommended gluing candy to a cardboard box framework with an eggwhite-based frosting, "the kind that dries into something resembling sheetrock". She included pictures of the gingerbread house made by her grandchildren, but you can find other patterns or ideas with an Internet search engine.

For my child, a big point is how to not get the candy in the first place, since he has special health requirements that mean he cannot have artificial food additives (especially dyes) or chocolate, poor thing. Some years we've gotten lucky with people giving treats that were things he could have, but this was not one of those years. He received two treats he could keep -- a container of applesauce, and a marshmallow. Rarely, we find white chocolate or organic candies, but don't really expect that. We are thrilled when people consider offering non-candy treats. Here are some ideas from good things he received in other years, and other non-candy ideas for treats. Remember to get things that are safe for children, and have a mix of items appropriate for a range of ages and genders.

Ask the Podcast Doctor - Live Blogging

Today saw the inaugural event in the Health Sciences Libraries lecture series. The speaker was Dr. David Stutz, a.k.a. the Podcast Doctor. Several people wanted to attend who asked where they could find the podcast, however (ironically) there was no podcast. As a wholly inadequate fill-in for the lack, please find below live-blogging from the event (which was in a room that lacked wireless network access also).

LISTENERS

Generate questions, such as "My wife says I have to eat more fish. Do I need to eat fish to have a healthy diet? Please say no!"

Two broad question types:
1) Immediate cry for help (refer to UMHS Physician Referral Service) "desperate questions"
2) general interest

As a physician is hard to be objective about what is interesting to general public.

Balance between depth of information and attention span.

Upload finished podcast to LibSyn.com - podcast service (Liberated Syndication)
Google Analytics
"Google Juice"

PROFESSIONALS

positive response

INTERNET & PT/CONSUMER HEALTH EDUCATION

InternetWorld stats 1.24 billion people use internet, ~20% of world population

medical consumers use internet as giant encyclopedia

biggest problem is too much information
how to filter and edit information
reams of printouts, most irrelevant to their personal problem

podcast not intended to be encyclopedic, more a general interest feature and entertainment value

unique in medical podcasts in that listeners generate/shape content

FUTURE

continue w/ 1/wk

FIG grant - fostering innovation
topic index for site

biggest problem is marketing
no promotion, but have 13,000 downloads

Q&A

1) How do you get people to use the site?
- UMHS homepage
- put site logo in UMHS screensaver for one week, will repeat later
- Aaron Block and Jessica Salero
- PR/outreach - site does not generate revenue; site subsidized by UMHS

equipment and domainname purchased out of pocket, not through institution

how to promote individual clinics? tear-offs?
presenters steer patients to content for pt education.

don't promote too heavily until there is enough content - about 100 est.

2) about questions

goaskalice as a similar type of site in "print" via web format

3) about accessibility or abstracting to enable searching of answers provided

haven't done that

4) why go to experts instead of answering questions yourself, as a generalist

suggested list of questions, semi-scripted
goal 1: answer question asked
goal 2: add relevant and useful information

monologue would not be interesting; dialog is more so

marketing is part of the concept
broader promotion for UMHS professionals

5) when selecting questions to answer, do you anticipate topics of interest?

no. timeliness is serendipitous rather than intentional.

6) is included more info? followup of further reading information?

yes, refer to UMHS health topics pages

7) How is that UMHS Health Topics content selected?

Don't know. He usually uses sports medicine info which does not go out of date quickly.

Ethics and Legalities in Dentistry - The Case of the Dentist Joker and the Assistant's Pigs

A really bizarre case has appeared in the legal literature. In this case a dentist performed surgery on one of his assistants who was also a patient. The assistant had a hobby or outside interest of caring for pot-bellied pigs. The surgery included the placement of temporary implants. As a practical joke, while the assistant/patient was under anesthetic, the dentist placed temporary implants of boars teeth, took photos, and then removed the boars teeth and continued with placing the actual temporary implants.

As you might have guessed, when the assistant saw the photos, she did not find it at all funny, which was the beginning of the legal cases. The dentist expected that his insurance company would cover him when he was sued, however they declined. He settled out of court with the assistant, but then sued his insurance company for not covering him. He won. He not only won, but he was granted the entire sum of money he paid the assistant plus three times that for mental distress.

This looks to be a very interesting case. More details and information are included in the following two FindLaw articles.

"For Each Child the Mother Loses a Tooth" - Where Did it Start?

In 1740, Joannus Baptista Mongin wrote a short (7 page) work describing pregnancy as a risk factor for tooth loss. You may remember the old saying, "for every child, the mother loses a tooth"? Well, perhaps this is where it began. The original book is in the Rare Book collections of the Dentistry Library, but for those who would like to take a look at this without coming to the library, here are images of the original pages in Latin.

New Exhibit: Willoughby D. Miller

The Dentistry Library is pleased to announce, in partnership with the Sindecuse Museum of Dentistry, a new exhibit on the life and research of Willoughby Dayton Miller. These exhibits are mounted in recognition of the 100th anniversary of Dr. Miller's untimely death.

WD Miller was hired to be the second dean of our School of Dentistry, having been recruited by Neville Hoff. Dr. Miller died shortly after his arrival in this country, having met with the faculty to discuss hopes for the future of the school, but never having the opportunity to implement any of these plans. Why was Miller so aggressively recruited? Because at that time, he was the very best known scientist in all of dentistry, having discovered the relationship between oral microflora and caries.

Miller's research was largely focused on saliva, but is still cited widely today. Recent articles citing his work appear in journals on dentistry (of course), surgery, obstetrics, microbiology, medicine, cancer, pathology, military medicine, and more. Some of the cancer articles are related to recent work on salivary diagnostics. Miller is one of those astonishing researchers whose work has immediate relevance far beyond the culture and times in which he lived and worked.

The Sindecuse Museum exhibit cases will highlight the life and times of Dr. Miller, with more information forthcoming from the Museum. The Library portion of the exhibit highlights research done by Dr. Miller that is still influential today. Both exhibits will remain on display through the end of 2007. A web-based version of the exhibit is forthcoming.
