A DVD of our Mission Trip will be produced when we return from traveling. We ask that all submissions to be considered for use in theproduction of the DVD be submitted not later than 14 days from the date of return.

If you would like to have something considered for use and so not believe you will be able to supply it within those 14 days, please contact your team leader and other arrangements wil be made.

We encourage you to submit any images, video footage, written statements, devotional, prayers or any other information you may feel will be helpful in creating a testimonial memory of our trip.

Submission Guidelines:
* Originals will not be returned. Submit copies only.
* Photo CD's are preferred.
* DVD/Video submissions must include a description of desired footage. If no description is submitted, discretion is left to the producers.
* This release form must be included with your submission.

Release Consent:

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby consent to the photographing of myself and recordings of my voice and the use of these photgraphs and/or recordings singularly or in conjunction with other photgraphs and/or recordings for advertising, publicity, commerical or other business purposes, as well as for the purpose of volunteer recruitment, placement and communication. I understand that the term "photograph" as used herein encompasses still photgraphs and motion picture footage including but not limited to DVD and video production.
